No wonder SEC could not find $50B fraud of Madoff. In the internal investigation report released to public in December found following things.

Mumbai police is saying there were 10 terrorists. But I suspect some more were there who got away. IB of India believes there were 20 terrorists.
I think I have the proof that some people got away with local help. Times of India food editor Sabina was stuck in her room in Taj on 26′th. She was sending text messages [...]
Media hindered NSG ops

During Mumbai attacks Indian TV stations were showing live coverage of the events unfolding. Media raced to show the stories live. But they did not consider that the people watching them may be terrorists, their accomplice. Every attack position of the NSG was known to terrorist helpers. Mumbai terror attacks have shown the weak links in the Indian intel. Now the blame game is going around between intelligence agencies, state police, customs officers and coastgaurd.
